Street-Based Services

Meeting people where they are, building trust, and navigating the process of moving from street to shelter is the role of our Outreach team. They are the first line of help during natural disasters and public health emergencies.

Emergency Shelters & Interim Emergency Housing

We operate congregate shelters, interim housing, and seasonal weather shelters providing safe shelter for up to 1,000 adults and families a night. 

Permanent Supportive Housing

With case management, employment navigation, and other programs, we can support individuals, youth, and families who have experienced homelessness, or are at risk of homelessness, stay housed.

Veterans Support

With over a decade of experience, HomeFirst offers the most comprehensive services for homeless Veterans—including mental health counseling, emergency shelter, and permanent housing.

Prevention Programs

Staying housed is the most important way to prevent homelessness. We have several programs that provide financial help to support at-risk people and families.

Street-based Services & Emergency Shelter

Our street outreach team and emergency shelter provide support and services that result in successfully transitioning 1-in-2 people from unsheltered homelessness to permanent housing.

Coordinated Entry

HomeFirst manages the county-wide system to assess the needs of every person experiencing or at risk of homelessness and effectively connect them to housing solutions.

Rapid Rehousing

By providing incentives to landlords and support to program participants, this program helps people experiencing homelessness transition into permanent housing. 

Prevention Programs

Keep People Housed is a Sonoma County program that provides temporary financial assistance and services to people at high risk of homelessness or housing loss.
