Street Outreach & Emergency Shelter

Through a consistent, compassionate presence, we connect people with immediate resources—food, hygiene, medical and mental health care, and a pathway to safe shelter.

Emergency Interim Housing

An evidence-based, trauma-informed care design that provides private sleeping cabins, case management, and mental health support within a supportive community that enables people to stabilize and move to permanent housing. 

Veterans Programs

With over a decade of experience that has helped define the national standard of care for homeless Veterans, our comprehensive, evidence-based approach provides the resources and support Veterans—and their families—need.

Permanent Housing

By combining affordable housing with ongoing, individualized on-site support—mental health, employment assistance, and life-skills—we help people build the stability, confidence, and connections they need to thrive.

Coordinated Community Care

By creating one community-wide pathway to housing and services, people are assessed to identify their unique needs and be connect to the right resources—whether that's emergency shelter, rapid rehousing, or permanent supportive housing.

Street Outreach & Emergency Shelter

Our street-based outreach teams connect with people where they are experiencing homelessness—in encampments, on the streets, or in their cars. They build trust, offer support, and connect people to shelter, housing, and supportive services.

Interim Housing

Interim housing communities offer private sleeping units, shared community spaces, and access to on-site support services and are a critical step toward permanent housing.

Veterans Programs

As the largest supporter of Veterans in Santa Clara County, we have piloted many programs that have become the national standard of care. Using this experience, we have developed a broad continuum of care that meets this population's unique needs.

Permanent Housing

By providing supportive services combined with long-term housing subsidies, we offer all the necessary support for people experiencing chronic homelessness to achieve and maintain housing stability.

Systems Leadership

We lead Coordinated Entry—the community-wide system that connects people to housing. We assess each person's needs, match them to the right housing solution, and coordinate with community partners to ensure no one falls through the cracks.
