We provide compassionate, end-to-end services, and have the expertise to support people in Sonoma County experiencing homelessness. Our Housing First, evidence-based approach removes as many barriers to housing as possible.
Sonoma County Programs
Systems Leadership
We operate Sonoma County's Coordinated Entry System—the unified pathway matching people experiencing homelessness to housing, prioritizing those with the highest vulnerability and need. We analyze data to drive strategic resource allocation, identify service gaps, and train partner organizations to ensure consistency to everyone seeking help.
Central to the system is a real-time county-wide database of every person currently experiencing homelessness—the By-Names List. Weekly case conferences bring together all community stakeholders to review each individual, develop housing plans, and monitor progress.
Prevention
We are part of the county-wide Homelessness Prevention System which operates in collaboration with community partners. By providing temporary financial assistance and case management, we can provide people with needed housing support.
Outreach
Outreach teams are a critical bridge between people living unsheltered and the Coordinated Entry System. The teams locate people living unsheltered who are eligible for support services and connect them with the appropriate help. By meeting people where they are, we are able to build trust, provide vital information about available housing opportunities, and facilitate pathways to housing. Our teams also provide critical support by completing needed documentation for housing placement.
Interim Housing
Labath Landing is a time-limited interim housing program designed to stabilize people experiencing homelessness. Program participants have private units with a locking door and 24/7 on-site support staff. There is a community kitchen, showers, laundry, and a computer lab. Case managers develop an individualized housing action plan tailored to a person's goals, and workshops support strengthening life skills and community reengagement.
Permanent Housing
Permanent Supportive Housing (PSH) is an evidence-based housing program that gives people experiencing chronic homelessness—especially older adults and those with disabilities—a stable home and the support they need to stay housed. We provide supportive services across the county at numerous housing sites.
Rapid Rehousing
In 2025, we launched Sonoma Valley's first permanent housing program for people experiencing homelessness. By partnering with the County of Sonoma, the City of Sonoma, and local landlords, we provide housing navigation and support so that people can quickly find and maintain housing stability.
Special Populations Program
Working with the City of Petaluma, we are improving outcomes for specific subgroups. Right now, the focus is on families. Once identified, the program coordinates all the systems of support to ensure stability and safe housing.

In 2025, we showed up for our community. From Veterans sleeping in their cars to families at risk of homelessness to individuals who had nowhere else to turn.
